What is a UDID card?
The Unique Disability ID (UDID) card is a single national identity card and disability certificate for persons with disabilities. It is valid across India and is used to claim pensions, scholarships, reservations, travel concessions and other benefits. With the 2026 revision of the central disability pension, which now covers people with 40% or more disability from birth, a UDID card is the easiest way to prove eligibility.
Who can apply
Any person with one of the 21 disabilities recognised under the Rights of Persons with Disabilities Act, 2016 can apply. Parents or guardians can apply on behalf of a child. Applying is free.
Step by step
- Go to swavlambancard.gov.in and choose Apply for Disability Certificate and UDID Card.
- Fill in personal details, disability details and contact details.
- Upload a recent photo, signature or thumb impression, and identity and address proof. If you already hold a disability certificate, enter its details.
- Submit the form and note your enrolment number.
- Your application goes to the Chief Medical Officer or medical authority of your district, who will call you for assessment.
- After assessment, the disability certificate is issued and the UDID card is printed and sent by post. You can also download the e-UDID card from the portal.
Track your application
Use Track Your Application on swavlambancard.gov.in with your enrolment number or Aadhaar number. If the card shows wrong details, you can request an update on the same portal.
Tips
- Keep medical records and prescriptions ready for the assessment.
- The percentage of disability on the certificate decides eligibility for many schemes, so attend the assessment yourself.
- Never pay an agent: the application is free.
